Services for Children in both age groups: 0-5

A note of explanation: This website focuses on children ages 0 to 5. This section refers to services available within that age range -- this does not mean that services end at age 5. With the exception of HRIIC Round Table (which serves only children ages 0-5), the services below can be available beyond age 5.

California Children Services (CCS)

CCS provides services to eligible children under the age 21 who have a serious medical condition. Their services include:

  • diagnostic evaluations suspected of having one of the CCS eligible medical conditions;
  • treatment services for the handicapping condition, including:
    • physician services;
    • hospital and surgical care;
    • physical and occupational therapy;
    • laboratory tests;
    • x-rays;
    • appliances;
    • equipment; and
    • other needed services.
  • physical and occupational therapy provided in medical therapy units located in public schools for children with physical handicaps who have a medical need for these services;
  • medical case management, which includes:
    • referrals to specialists and centers;
    • follow-up with other involved in care;
    • transferring medical records;
    • locating new facilities and services when family moves.

San Francisco Community Behavioral Health Services (SFCBHS)

SFCBHS services include for children 0-5 include:

  • assessment;
  • evaluation;
  • counseling; and
  • mental health consultation in child care centers.

Support for Families of Children with Disabilities (SFCD)

SFCD provides services (free of charge) to families of children with special health care needs of all ages, and to professionals who serve those children. Their services include:

  • phone line and drop-in center at Open Gate;
  • lending library;
  • support groups;
  • Parent-Mentor program;
  • parent/professional workshops;
  • educational clinics;
  • Individualized Education Program (IEP) workshops;
  • parent teams/community outreach;
  • Family Links to Mental Health;
  • Child Care Inclusion Challenge Project;
  • newsletter.

HRIIC Round Table

The Round Table is an interagency process for exploring which agency or agencies might best meet the need of children ages 0-5 with special health care needs. This is done by:

  • Interagency review to coordinate referrals for children found ineligible for services from GGRC, CCS or SFUSD, but for whom there is a concern that the child needs supports or services;
  • Interagency review for children who may be eligible for services from an agency, but for whom there is additional concern/need that other agencies may be able to meet;
  • Quarterly reviews to ensure that connections were made between the family and agency.
