Medi-Cal is a PUBLIC PROGRAM. Child Health and Disability Prevention Program (CHDP)Telephone: 415/575-5712 CHDP provides early and periodic screening, diagnosis and referral for potentially handicapping conditions for children and youths. CHDP promotes preventive health services and assists families in obtaining appointments for regular assessments and diagnostic/treatment services as needed. Local public health departments assist CHDP providers in tracking and monitoring families to assure that appointments are made and kept. All persons under 21 years of age who are eligible for Medi-Cal qualify for CHDP services. CHDP is a PUBLIC PROGRAM. Healthy FamiliesTelephone: 888/774-1222 Healthy Families provides low cost health coverage for eligible children under the age of 19. Services include regular health check-ups, immunizations, labs and x-rays, prescription medicines, hospitalizations, dental and vision care, substance abuse and mental health services. Healthy Families is a PUBLIC PROGRAM. It is California's (SCHIP) program. Healthy Kids568 Howard Street Healthy Kids provides low cost health coverage for San Francisco children under the age of 19 who meet eligiblity requirements. Coverage includes full medical, dental and vision care services. Families who earn too much to qualify for other low or no-cost health coverage programs may qualify for Healthy Kids. Healthy Kids is a PUBLIC PROGRAM. Other Education ServicesEarly Head Start777 Stockton Street, Suite 202 Early Head Start provides education, health, mental health, nutrition and social service to pregnant women and children ages birth to three years old. Early Head Start offers services through home-based and center-based programs, as well as family childcare. Early Head Start is a PUBLIC PROGRAM. Head StartAddress: 205 13th St. Head Start provides a learning environment for eligible children. The program provides experiences that support the continuum of children's growth and development, which includes the physical, social, emotional, language and cognitive development of each child. Head Start is a PUBLIC PROGRAM Family Assistance ProgramsCalWORKS170 Otis Street, 1st floor Eligible adults with dependent children receive financial support and a full array of services for 18-24 months as they work with an Employment Specialist to follow an individualized Employment Plan. This plan may include job preparation, assessment, training, counseling and treatment, education and job search activities which lead to a job and self-sufficiency. Child care is paid for CalWORKS participants. Clients also receive Food Stamps, Medi-Cal and Fast Pass or tokens, and may qualify for a housing subsidy. CalWORKS is a PUBLIC PROGRAM. Food StampsMain office: 1235 Mission Street Eligibility is determined by household income. Food Stamps coupons can be used to purchase food items. Participants can pick up their coupons every month at locations throughout the city. Food Stamps is a PUBLIC PROGRAM. Women, Infant and Children Supplemental Nutrition Services (WIC)30 Van Ness Avenue, Suite 220 WIC is a nutrition program for women who are low-income, pregnant, breastfeeding/postpartum, infants and children up to five years of age. The WIC Program provides supplemental foods (such as milk, cheese, cereal, eggs, beans, peanut butter, and juice), breastfeeding and nutrition education, and referral to health care. WIC is a PUBLIC PROGRAM. Supplemental Security Income (SSI)Telephone: 800/772-1213 Supplemental Security Income (SSI) is provided through the Social Security Administration. SSI provides monthly stipends and access to Medi-Cal for children with specific disabilities and chronic illness. SSI is a PUBLIC PROGRAM. |
